Appraisal Waiver Requirement Change?

I received appraisal waivers for the refinances I completed last year (< 1M house value, <60% LTV).  I just applied to the same lender and they are saying that desktop underwriting is now asking for an appraisal.  Did something change with the requirements to get an appraisal waiver?

Nothing changed to my knowledge I'm still getting appraisal waivers with the same inputs.
