"Clinical Considerations: Myocarditis and Pericarditis after Receipt of mRNA COVID-19 Vaccines Among Adolescents and Young Adults:

Consider myocarditis and pericarditis in adolescents or young adults with acute chest pain, shortness of breath, or palpitations. In this younger population, coronary events [heart attacks] are less likely to be a source of these symptoms."  i.e. vaccine caused myocarditis/pericarditis is more likely cause and that for younger population, a heart attack is not the usual cause. "For suspected cases [of myocarditis/pericarditis], consider consultation with cardiology for assistance with cardiac evaluation and management.  For follow-up of patients with myocarditis, consult the new guidance from the American Heart Association."

Follow-Up:  It is prudent to perform regular cardiology follow-up incorporating electrocardiography, echocardiography, and laboratory tests at a frequency similar to that for recently diagnosed acute heart failure from cardiomyopathy. ...Because children are active and many will recover functional capacity [functional capacity is an estimate of what the patient's heart will allow the patient to do], exercise and activity restrictions demand special consideration....Patients should not participate in competitive sports while active inflammation is present....
Conclusions:  Myocarditis in children remains a challenging condition to diagnose and manage. Its impact on lifelong morbidity and mortality is significant."

Post vaccine myocarditis is a rare complication and tends to be mild and self limited. Any medication, whether its a blood pressure pill or vaccine has risks and benefits. In the case of MRNA COVID vaccines, they have been show to be quite effective in reducing death and hospitalization. While there has been much media attention about these rare complications, it should be taken in the context of a pandemic that has killed ~800,000 in the US. Abstaining from the vaccine will significantly increase your risk for getting COVID and expose you to short and long term cardiovascular complications.

"One study estimated that COVID-19 vaccination of the group of individuals who were most likely to experience post-vaccination myocarditis (adolescent and young adult men) would prevent >10 times the number of hospitalizations and 3–4 times the number of ICU admissions owing to COVID-19 than would result from cases of vaccine-associated myocarditis."
