ENT for sinus issues

Hello.

Does anyone know of a good ENT surgeon for chronic sinusitis treatment? I've had surgery to clean them out back in NJ, but they have flared up big time in the last few months.

I had seen this ENT to check my ears out after doing a lot of diving.  I really liked him.  He had an office in Irvine near Culver and Barranca but he may have moved to Santa Ana.  Here is what I found via Google:
